Medications Travellers' diarrhoea is the most common travel-related ailment. The cornerstone of prevention is food and water precautions, as outlined below. All travellers should bring along an antibiotic and an antidiarrhoeal drug to be started promptly if significant diarrhoea occurs, defined as three or more loose stools in an 8-hour period or five or more loose stools in a 24-hour period, especially if associated with nausea, vomiting, cramps, fever or blood in the stool. A quinolone antibiotic is usually prescribed: either ciprofloxacin Cipro ; PDF ; 500 mg twice daily or levofloxacin Levaquin ; PDF ; 500 mg once daily for a total of three days. Quinolones are generally well tolerated, but occasionally cause sun sensitivity and should not be given to children, pregnant women, or anyone with a history of quinolone allergy. An antidiarrhoeal drug such as loperamide Imodium ; or diphenoxylate Lomotil ; should be taken as needed to slow the frequency of stools, but not enough to stop the bowel movements completely. Diphenoxylate Lomotil ; and loperamide Imodium ; should not be given to children under age two. Most cases of travellers' diarrhoea are mild and do not require either antibiotics or antidiarrhoeal drugs. Adequate fluid intake is essential. If diarrhoea is severe or bloody, or if fever occurs with shaking chills, or if abdominal pain becomes marked, or if diarrhoea persists for more than 72 hours, medical attention should be sought. Though effective, antibiotics are not recommended prophylactically i.e. to prevent diarrhoea before it occurs ; because of the risk of adverse effects, though this approach may be warranted in special situations, such as immunocompromised travellers. Background: People with epilepsy experience difficulty in obtaining and retaining work, but little is known about the experiences of working with epilepsy. This qualitative study aimed to gain insight into patients' experience of working with epilepsy. Methods: 35 participants with epilepsy, currently employed, were interviewed using a semistructured format. A theoretical sampling frame was used to ensure a representative sample by gender, seizure frequency, and job type. All interviews were tape-recorded and transcripted. Content analysis was used to identify emerging themes. Results: A wide range of occupations were represented. The median age at the time of diagnosis was 14 years range, 1-48 years ; and the median duration of epilepsy was 17 years range, 1-51 years ; . Some of the main themes identified were work factors, disclosure, and issues related to driving. Several subthemes within work factors also emerged including attitudes of work colleagues and the patients, career progression, and the need for job modifications. Some persons experienced few problems, whereas for others there were major problems, which included a change of job. The impact of epilepsy at work related mainly to effective control of seizures, the type of job, attitude of the employer, and inability to drive. Conclusion: The impact of epilepsy at the workplace was variable for subjects. For some of those who experienced problems, it had forced them to modify work practices or to change job or had a negative effect on their career progression.

WASHINGTON, May 21 UPI ; -- Mounting evidence suggests the anti-malaria drug Lariam -- prescribed to Peace Corps volunteers, travelers and U.S. soldiers -- has triggered mental problems so severe that in a small percentage of users it has led to the ultimate side effect: suicide. Lariam -- also known as mefloquine -- is a product of Hoffmann-La Roche, a giant Swiss pharmaceutical company with U.S. headquarters in Nutley, N.J. Lariam has been prescribed to more than 22 million people worldwide since 1985. It was cleared for use in the United States in 1989. Some health experts charge that neither patients nor doctors in the United States are being adequately warned about the risk of suicide from taking Lariam, which is prescribed by U.S. doctors 1, 000 times every day. William Manors, who became severely ill after taking mefloquine in Iraq and Kuwait while deployed for Operation Iraqi Freedom. Another soldier with a mefloquine diagnosis is a Green Beret who served in Afghanistan. UPI reviewed a copy of Menisci's medical report from the San Diego lab, which includes the notation, "Lariam induced, " with the word Lariam underlined. Earlier this month, Manors filed suit against Lariat's manufacturer, Swiss drug giant Hoffmann-La Roche, for alleged failure to warn of the drug's risks and marketing a product it knows is unsafe. Manors said he became mentally and physically ill after taking the drug, at one point taking his gun apart because he was afraid he was going to kill himself.
