Chronic hepatitis B virus HBV ; infection is a leading cause of cirrhosis and hepatocellular carcinoma worldwide. Its prevalence approaches 10% in hyperendemic areas, such as southeast Asia, China, and Africa. Although chronic HBV infection is seen less frequently in North America and Europe, an estimated 1.25 million persons in the United States are infected. In the past decade, revolutionary strides have been made toward the treatment of chronic HBV infection. Interferon- was once the only available therapy but has recently been joined by the nucleoside analogues, the most extensively studied of which is lamivudine. Interferon therapy continues to have a role in the treatment of a carefully selected group of patients. Lamivudine therapy, which has less stringent selection criteria, suppresses HBV DNA in almost all treated patients: Seventeen percent to 33% experience loss of hepatitis B e antigen, and 53% to 56% have a histologic response. Extended lamivudine treatment leads to the development of a specific lamivudine-resistant virus with base-pair substitutions at the YMDD locus of the DNA polymerase. Newer nucleoside analogues and other immunomodulator therapies are being investigated. In the future, combination therapy with different classes of agents may yield improved response rates and delay the development of resistance.Present Situation: According to the American Academy of Child and Adolescent Psychiatry, psychiatric medication is an important part of treating certain psychiatric disorders in children and adolescents but should be used only as one part of a comprehensive treatment plan with ongoing medical assessments and in conjunction with other services such as individual and family therapy. Medication may be prescribed for psychiatric symptoms and disorders, including, but not limited to: anxiety, attention deficit hyperactivity disorder, obsessive-compulsive disorder, depressive disorder, eating disorder, bipolar manic-depressive ; disorder, psychosis, bedwetting, sleep problems, autism, and severe aggression. IHEC was invited to participate in the Service Learning Fair at Walter Payton HS in September 2005 to promote our Hepatitis Education and Prevention Program HEEP ; High School Peer Educator Project. Following the fair, a Payton student contacted HEPP to be trained as a peer educator. Working with that student and the Service Learning Coordinator at Payton HS, Cassandra Stiff, an informational meeting was held for other interested peer educators. Five dedicated students attended that meeting and have become the Payton HS Hepatitis Peer Educators. "At schools with low immunization compliance rates, the peer educators are able to assist the school nurse and HEPP School Outreach Coordinator in planning and promoting immunization drives, " Carla Green, School Outreach Coordinator, said. The HEPP Peer Educator Program has been implemented in various Chicago Public Schools since 2001. Through their participation in the program, the students earn Service Learning credit hours, which are required for graduation from all Chicago Public Schools. The initial meetings of the Payton HS Peer Educators consisted of trainings for the students on viral hepatitis and Vaccine Preventable Diseases. At one of the sessions, four students from the University of Illinois at Chicago College of Medicine APAMSA organization conducted the presentation. These students were among the 22 students at UIC that were trained by HEPP in Fall 2005 to perform hepatitis education in high schools. Conventional medical wisdom now endorses the use of SSRI's as a first line drug in the treatment of depression in the elderly. The safety and efficacy of these medications has been demonstrated in patients over the age of 65 as well as those with significant cardiovascular disease or individuals with complicated medical problems such COPD and cancer. These medications are effective in lowering anxiety in patients; however, they are not as effective for treating chronic pain as the tricyclic antidepressants. Nortriptyline Pamelog ; is the most effective tricyclic antidepressant with the least anticholinergic side effects. Doses beginning from 30-50 mg per day can be used at bed time to assist sleep. Blood levels should be checked every week during dose adjustments and the dose titrated to a blood level of 70-100. The antidepressant should be continued for six weeks to determine efficacy. Avoid Elavil in the elderly since most patients over age 65 will become confused or delirious from the medication. Patients with significant cardiac conduction problems may be at risk for heart block with tricyclic antidepressants. Overall, therapeutic doses of Pamelor are well tolerated by even the frail elderly patients since Pamelor does not disrupt sleep or appetite. A nighttime dose may lessen sleep disturbance. This study was approved by the Research Subjects' Protection Program at the University of Minnesota and by the Scientific Advisory Committee of the General Clinical Research Centre CRC ; .
