DISCUSSION In this study, we have demonstrated the formation of TAM-DNA adducts in a monkey model using two methodological approaches: TAM-DNA CIA and HPLC-ES-MS MS. Monkeys were exposed to 2 mg of TAM kg bw daily for 30 days. The dose used 2 mg of TAM kg bw day ; was 6-fold higher than the current recommended daily dose for women. However, the monkeys in this study were only exposed for 1 month, whereas breast cancer patients typically receive TAM for 5 years. TAM-DNA adducts were found in most tissues examined, with the highest levels being detected in the liver and brain cortex. Although TAM-DNA CIA detected adduct levels of 0.72.1 108 nucleotides in kidney DNA samples, lower levels 0.2 0.4 adducts 108 nucleotides ; were measured by HPLC-ES-MS MS. If the major kidney DNA adduct was dG-desmethyl-TAM, it would not be detected by HPLC-ES-MS MS at the levels indicated by TAM-DNA CIA. Most importantly, TAM-DNA adducts were found in the uteri and ovaries of all three exposed animals. The data demonstrate that nonhuman primates are capable of metabolizing TAM into genotoxic intermediates that form DNA adducts in suspected target tissues. The data also suggest that humans may have the metabolic capacity to form TAM-DNA adducts in organs at risk for TAM-induced cancers. In addition to the TAM-DNA CIA and HPLC-ES-MS MS analyses reported here, DNA samples from this study have been assayed by 32 P-postlabeling-HPLC by Shibutani et al. 27 ; . The method differentiates between dG-TAM and dG-desmethyl-TAM and determines both adducts with similar sensitivity. The epidemiology of viral hepatitis during pregnancy is essential for health programme managers. Also, identification of the infected pregnant women to HBV allows prevention of newborn infection by active and passive immunization soon after birth. This study was designed to assess the prevalence of HBV and HCV infection among Iranian pregnant women. Serum samples from 496 pregnant women, who lived in rural 35.1% ; and urban areas 64.9% ; of Iran Jan. to July. 2007 ; , were collected. Data were obtained through questionnaires distributed during pregnancy. The mean age of this population was 25.85.2 years and mean period of pregnancy was 4.8 months. Samples were first screened for anti-HBc and anti-HCV antibodies by Third generation enzyme linked immunosorbent assay and those found positive for antiHBc antibody were further tested for HBsAg. Statistical analyses were done to explore the association of demographic variables with anti-HCV and anti-HBc antibodies. Anti-HBc was found in 23 of the 496 women overall prevalence, 4.6%; 11 of 166 from rural areas, 6.2%; 12 of 307 from urban areas; 3.8% ; . Of these 23 positive samples, 5 2.1% ; were positive for HBsAg. Only one sample 0.2% ; was anti-HCV positive. Anti-HBc prevalence in rural was higher than urban areas but this difference was not significant. The presence of anti-HBc antibody is a marker of exposure to HBV. Therefore, the present study establishes HBV infection rate in a specific group of women. These results emphasize the need for prenatal screening for HBV infection in pregnant women and treatment of the newborns from HBsAg positive mothers. In the case of HCV, more studies are needed to identify risk factors of HCV infection and the importance of HCV screening and treatment programmes. Carotid stenting offers the potential of nonsurgical treatment of carotid bifurcation stenoses with possibly lower morbidity than surgery in those patients in high surgical risk categories. Reliable evaluation of the outcomes of carotid stenting is limited at this time because the outcomes have been reported in multiple case series with few randomized trials, and the outcomes are heavily affected by patient selection. In addition, the long-term durability of carotid stents in preventing stroke and the long-term patency rates of carotid stents are not yet established.
