Background: Among communicable diseases, tuberculosis TB ; is a leading cause of death worldwide, killing 2 million people each year. TB in Taiwan is a major public health issue as well. In 2004, the mortality and incidence rates of TB in Taiwan were 4.2 and 74 per 100000 population, respectively. Compared to the TB mortality and incidence rates in the United States 0.5 and 5 respectively ; and Japan 4 and 30 respectively ; in 2004, there is much room for Taiwan to improve TB control. A major system redevelopment project in 2001 marked the beginning of a continuous effort in converting the existing system to be web-based. Web-based modules developed to-date include case notification, case management for hospital personnel, automatic indicator-based monitoring of total quality management, automatic alert via cell phone to designated health officials, management of medications, and online professional development. Objective: The objective was to evaluate a web-based National TB Register system designed for health care professionals to report accurate, timely and reliable data as part of TB surveillance and case management. Methods: The performance of this system was evaluated in terms of improvements in follow-up of TB treatment cohorts and the compliance of notification timeliness. Results: The enhancements have resulted in significant system usage. Currently, this system is used by 832 institutions, 5128 individuals in Taiwan with an average of 210000 logins annually. With implementation of internet tools, improvements in the following areas have been observed: i ; the trend for the reduction of defaulted and transferred out patients in 2001-2003 treatment cohorts, ii ; the trend for the increase of compliance of notification timeliness 34% in 2001, 84% in 2002, and 99% in 2003, P .0001 ; . Conclusions: TB surveillance and case management involve a large number of specialized personnel and public health agencies. While Internet-based tools have been shown to effect improvement in TB outcome, it is a major challenge, from a system development of view, to understand users' requirements and to build a consensus among all users as to what tools and associated priorities need to be developed.

In addition, similar long-term effects of vasopressin have been demonstrated in the thick ascending limb of Henle's loop. Specifically, long-term increases in circulating vasopressin level have been demonstrated to be associated with increased expression of the Na-K-2Cl cotransporter 24 ; and increases in transepithelial chloride transport in isolated perfused thick ascending limb 1 ; . This long-term regulatory process couples with the previously demonstrated short-term action of vasopressin to activate the Na-K-2Cl cotransporter 28 ; to mediate overall regulation of thick ascending limb NaCl absorption and countercurrent multiplication. Based on the results of the present study and on previous observations, it appears that PGE2, like vasopressin, has both long- and short-term regulatory actions in the thick ascending limb. Specifically, studies by Stokes 33 ; in the late 1970s demonstrated that PGE2 has an inhibitory effect on NaCl absorption in isolated perfused thick ascending limbs from rabbits, an observation which has been confirmed in mouse 6 ; and rat 17 ; . This effect is mediated by an inhibitory G protein 7 ; , consistent with the role of the EP3 receptor. This inhibitory effect is seen within a few minutes of exposure of the thick ascending limb epithelium to the PGE2, a response which is probably too rapid to be caused by increased abundance of the Na-K-2Cl cotransporter. Indeed, it has recently been suggested that short-term regulation of the Na-K-2Cl cotransporter occurs via trafficking of intracellular vesicles containing the transporter to the plasma membrane 30 ; . Recent studies in cultured mouse medullary thick ascending limb cells suggest that the short-term inhibitory effect of PGE2 is a result of an action of PGE2 to regulate Na-K-2Cl cotransport activity 23 ; . Thus there appear to be dual mechanisms for regulation of thick ascending limb Na-K-2Cl cotransport that are effective over different time frames. Thus far, it is not known and ampicillin. Both S. aureus and S. pneumoniae showed accumulation of many N-terminal formylated peptides proteins upon PDF inhibition. In S. pneumoniae, formylated peptide protein accumulation was time dependent. Following inhibition, subsequent removal of the inhibitor resulted in deformylation of formylated peptides proteins; this recovery process was also time dependent. If instead the inhibited cells were maintained in the presence of sub-MIC levels of the PDF inhibitor, the formylated peptides proteins remained for a much longer time, which correlated with a prolonged postantibiotic effect in vitro. These observations may have broader implications for the application of this class of antibiotics in vivo. With the emergence of resistance and cross-resistance to existing antibacterial drugs, agents with novel modes of action against pathogenic bacteria are in great demand. Over the past three decades, the only two antibiotic agents with novel mechanisms that have been brought onto the market are linezolid brand name Zzyvox ; and daptomycin brand name Cubicin ; . Inhibitors of peptide deformylase PDF ; are a new class of antimicrobials that have a distinctive mode of action and represent a unique opportunity in the antibacterial field 8, 10, 22 ; . PDF catalyzes the hydrolysis of formylated N-terminal peptides formyl-NH-peptide ; to generate free N-terminal peptides H2N-peptide ; during the process of protein maturation. The enzyme is essential for the growth of both gram-negative and gram-positive bacteria 14, 16, 17 ; . It is believed that all bacterial protein synthesis is initiated with N-terminal formylated methionine 18 ; . Eukaryotic PDF homologs have also been identified, and the mammalian PDF has been proposed as an anticancer target 7, 13, 19 ; . Unlike prokaryotic PDF, vertebrate homologs only exist in mitochondria and their exact functions are not clear. PDF inhibitors have demonstrated good selectivity toward bacterial cells versus mammalian cells, supporting this enzyme as a valid anti-infective target. Elucidation of the detailed mechanism of bacterial growth inhibition resulting from PDF enzyme inhibition not only will help to reveal how this class of antibacterial works, but also may provide insight into how to improve activity. PDF inhibitor LBM-415 also known as VIC-104959 ; was identified as an effective antibacterial agent and entered phase I clinical trials in preparation for its evaluation in the treatment of upper respiratory tract infections 5, 10, 11 ; . Aside from specific treatment of the MRSA infection, the practical question arises as to how to manage these cases in an outpatient setting. The major mode of transmission of MRSA in the hospital setting is via direct contact between patients and health care workers. Linezolid Linezolid Zyvox ; , the most promising recent release on the market6, is the first in the class of synthetic antibacterial agents called the oxazolidinones. It has shown consistently high activity against a wide range of Gram-positive pathogens, including staphylococci, enterococci including vancomycin resistant ; and Pneumococci.7 It is licensed to treat pneumonia and complicated skin and soft-tissue infection caused by Grampositive bacteria.8 Linezolid has a novel mechanism of action, preventing bacterial protein synthesis at an early stage; crossresistance between linezolid and other antibacterial agents is therefore unlikely.9 Linezolid has almost 100% bioavailability9 and can be given either intravenously or orally, allowing an intravenous to oral switch of therapy. It has been shown to have excellent tissue penetration.10 There have been rare reports of linezolid resistance in E. faecium but these have been limited to a small number of seriously ill patients treated for protracted lengths of time11 An isolated case of linezolid resistance to S. aureus was reported in July 2001 in a critically ill patient undergoing peritoneal dialysis.12 A case was reported at the beginning of 2003 in the UK13 and a further case reported in the US in September 2003.14 In Phase III clinical trials, linezolid was proven to be as effective as vancomycin in the treatment of nosocomial pneumonia15 and as effective as cloxacillin dicloxacillin in complicated skin and soft tissue infections16 Unavailable in the UK, cloxacillin is considered to be equivalent to flucloxacillin ; . Linezolid intravenous or oral has been compared with teicoplanin intravenous or intramuscular in 430 hospitalised patients with a variety of suspected or proven Gram-positive infections.17 Linezolid demonstrated a significantly better overall clinical success rate than teicoplanin at the end of treatment 95.5% vs. 87.6%; p 0.005 ; and in a subset of patients with bacteraemia 88.5% vs. 56.7%; p 0.009 ; .17 Study findings presented at Infectious Diseases Society of America IDSA ; meeting in September 2003 showed that clinical cure rates were significantly better in patients treated with linezolid for complicated skin and soft tissue. Japanese equivalent JP2558282 has received a 5 year extension for quinopristin, whilst divisional JP equivalent JP2659531 has received a 4 year, 3 month and 10 day extension for the combination. Equivalent US04798827 has received a 1770 day extension for Synercid. Equivalent AU601426 has been granted a five year term extension for synergistin derivatives cyclic depsipeptides ; and expires May 2012. In September 1997, Aventis Pharma formerly Rhone-Poulenc Rorer; RPR ; submitted an NDA with the FDA for a variety of Gram-positive bacterial infections including VREF, methicillin-resistant Staphylococcus aureus MRSA ; , methicillinresistant S epidermidis MRSE ; and drugresistant Streptococcus pneumoniae DRSP ; . Approval was recommended by an FDA Advisory Board; however, full approval was not granted by the FDA due to repeated good manufacturing practice violations by RPR's and Hoechst's joint venture production company, Centeon, which produces Synercid. The conditions specified by the FDA for full approval were met by Centeon in 1999 and in September 1999, the FDA approved the use of Synercid for the treatment of bloodstream infections due to vancomycin-resistant Enterococcus faecium VREF ; , and skin and skin structure infections caused by methicillin-susceptible S aureus. Synercid was approved in the UK in July 1999, for use in patients with nosocomial pneumonia, skin and soft tissue infections and clinically significant infections due to Enterococcus faecium when there is no other active antibacterial agent. Launch of it in the UK and the US followed in September that same year. Last week we reported on Japanese patent extensions granted to sanofi-aventis and to Daiichi now Daiichi Sankyo ; for Plavix and levofloxacin. On June 14, Daiichi announced that the Canadian Federal Appeal Court had affirmed the decision of the Federal Court in Toronto that Daiichi Sankyo's patent CA1304080 ; for levofloxacin was valid and infringed by Novopharm's generic levofloxacin tablets. As a result of this decision, Novopharm is enjoined from selling, offering for sale or otherwise dealing with a generic version of levofloxacin products including tablets in the Canadian market until the expiry of Daiichi Sankyo's patent on June 23, 2009. The Japanese Patent Gazette published May 25th also reported that two extensions have been granted to Ono Pharmaceuticals for landiolol, a very short-acting beta-1 adrenoceptor antagonist, for the treatment of tachyarrhythmia during surgery. To date landiolol only appears to have been launched in Japan. JP2004651 has been extended by five years, whist JP3302647 received almost 4.5 years JP3176630 received extensions of over 4 years, nine months for linezolid Zyvox ; which is indicated for the treatment of multiresistant Gram-positive bacterial infections including nosocomial pneumonia, skin and skin structure infections including diabetic foot ulcers ; and community-acquired pneumonia. Approval had been granted in Japan for the treatment of patients with vancomycin-resistant Enterococcus VRE ; infections in April 2001, on condition that Pharmacia now Pfizer ; collected data from all patients treated, as the filing was evaluated based only on foreign clinical data. In April 2006, the drug was granted approval for the additional indication of MRSA infection in Japan and it is for this indication that the current extensions, which expire June 2019, were granted. Worldwide sales of linezolid Zyvox ; reported by Pfizer for 2006 totaled 2.0 million, according to our Strategic Drugs database SDdb ; representing a 27% year-on-year growth and were expected by analysts to exceed billion in 2008.
